INSURANCE
I am now in-network for Pacific Source insurance plans and BCBS Texas PPO (please note I’m not in network for HMO plans). For all other insurance providers- this is a fee for service practice. If I do not take your insurance, I can provide you with an invoice that you may submit to your insurance for you to receive out of network benefits. Payment is required at the time of service.
Initial medication Assessment session
(60 minutes)- $400.00
First session, following an online intake completion to discuss recent issues and concerns. Initial evaluation is meant to start discussion for possible diagnosis and medication options to address symptoms. Two hour appts may be offered if needed with an additional charge of $50 per 15 mins.
psychotherapy
Session
(45 minutes) $350.00
For follow up session to set up psychotherapy. We can discuss different forms of therapy and your goals for therapy. This does not include assessment and evaluation for treatment with medications. $50 additional per 15 mins over 45 mins.
Medication Management session
(25 Minutes) $225.00
For established patients only. 25 minute follow up appointment to review medications for change or for refills. $50 additional per 15 mins over 45 mins.

For established patients only. A 45 minute follow up appointment with both medication management and therapy. $50 additional per 15 mins over 45 mins.
Phone Call/

(15 Minutes) $50.00
For established patients only. If a 15 minute phone call is needed outside of an appointment or if 15 minutes or more is needed to complete paperwork (FMLA, documentation for school, etc).
